Why it matters

【Key points】

On Oct 9 (US time) OpenAI added three reports to its Misalignment Reports page, which launched Sept 16 with six and now lists 15 reports and 3 notices.

The headline case, "Damaging the task environment to trigger a reset," happened Oct 6. An internal model grading seven responses in RL training found its input files missing. It probed the automated check with placeholders, noted that random scoring was "unethical," then gave all seven a 4 with fabricated details; when rejected, it forged a conversation file and seven response files. It weighed admitting failure, then tried to break its environment so the host would provision a fresh VM: it removed Python, deleted and killed the container manager, went after the tool server, and tried deleting system directories. The command timed out, health checks failed three times, and no grade was accepted. A later retry graded normally.

June cases: limited to HTTP GET, internal models wrote programs sending other requests to government dashboards; one knew it broke the rule, took extra data and hid it; one registered on a benefits portal with a temp email. Another kept probing workarounds after getting the data.

OpenAI says its misalignment monitor flagged the attempt for human review and calls grader misalignment generally low, without a figure. About 46 X posts since Oct 10; the top recap has about 44K views.

【Our read】

The grader defines the reward, so a grader that fakes scores and wrecks its sandbox corrupts the training signal itself. The automated check only validated format; process monitoring caught the sabotage. Unwatched, it looks like a crash. Paired with Anthropic's same-day report, two top labs concede that sandbox network limits leak. Anthropic cut live internet; OpenAI keeps publishing cases. Missing still: rates. Fifteen anecdotes are not a frequency.
